Each school will receive through district mail copies of a revised form for reporting injuries and accidents to pupils. To avoid confusion, please clear your files of any unused copies of the old form. The new version is somewhat more detailed but not excessively so.
On applicable accident injuries, the form should be prepared in duplicate with the original form for the district office and the carbon for the school. It is essential that these be used, and all teachers and employees should be so informed and expected to do so.
A written report should be filed in situations such as the following:
A. In all cases where the student had to be removed from activity or from school for more than an hour or two.
B. In all instances where a physician was consulted.
C. In most instances requiring first aid. If the injury is a minor bruise, abrasion or laceration requiring first aid the teacher should make a written note of the facts related to the injury and leave it in the first aid room for about ten days. If at the end of that time the injury has healed satisfactorily, the note should be destroyed and no report of the injury filed.
D. In all instances where there was involved a defective piece of equipment or a hazard in the equipment or the school plant that needs correction.
