Licensed Employees - New Employee Information

Welcome to Logan City School District! We are excited to have you as a member of our educational team and look forward to working with you this upcoming year. In order to finalize the hiring process, we will need a number of required documents completed.   Please be advised employment may not begin and no paycheck will be issued until all paperwork has been completed and returned to the Department of Human Resource.   

Please print, complete, and bring all required documents to the Department of Human Resource.   
When you return these documents for processing, Licensed Employees will meet with Lindsay Auld or Stephanie Adams.  Please plan on 20 minutes to begin the hiring paperwork process.

I-9 Document from Homeland Security (Must be downloaded and opened in Adobe Acrobat. Will not open in your browser or Apple Preview)

W-4 Form from IRS
