Vision Screening
The District shall conduct free Tier 1 and Tier 2 vision screening as described below for all qualifying children enrolled in the District.
- Tier 1 vision screening shall be conducted for students in pre-K, Kindergarten, grades 1,3,5,7, and grade 9 or 10.
- Tier 1 vision screening shall be conducted for any student referred by school personnel, parent/guardian or self to rule out vision as an obstacle to learning.
- A Tier 2 screening shall be done for student(s) requiring education intervention such as Special Education referral or re-evaluation or failing benchmark reading assessments as defined by USBE Rule R277-404 or if indicated by the results of the Vision Symptom Questionnaire. If a student has already been referred to an eye care professional in after failing a Tier 1 screening, the Visions Symptom Questionnaire and Tier 2 screening are not required.
- The District may automatically refer the student to an eye care professional in lieu of performing a Tier 2 vision screening.
- A Tier 2 vision screening may be done based on parent/guardian or teacher concern.
- Student(s) with an Individualized Education Plan (IEP) will be screened and referred in accordance with Department of Health and Human Services R384-201.
- A volunteer who serves as a vision screener for a free vision screening clinic for a qualifying child must be a school nurse, hold a certificate issued by the Division of Services for the Blind and Visually Impaired, or be directly supervised by a school nurse or individual with a. certificate issued by the Division.
